New Patient Registration

How to Register

Please note that now new patients have the facility to register online.

You can also register in person as before. New Patient registration times: 11.30am to 12.30pm and 2pm to 4pm.

When you attend the Practice to register you will be asked to complete a GP Registration Form, and New Patient Questionnaire Form; these must be completed before you see the Doctor, so it is advisable to attend at least 10 minutes before your appointment time.

There are a number of reasons why you may not be able to register with your chosen GP, for example you may live too far away. If this is the case simply choose another GP in your local area.

Please note that your medical records may take several weeks to arrive at the Practice.

Catchment area

We can only register patients who currently live within our catchment area. Please enter your postcode below to check if your address is within our area.

Register online

We are using an NHS online service called Register with a GP surgery that makes it easy to register with this GP surgery.

Just fill in this quick online form to start the process. You do not need proof of address or immigration status, ID or an NHS number.

The service is designed and run by the NHS, so your personal information is safe. It reduces our administrative workload and makes it easier for you to register.

Paper forms are still available if you need one.

The National Care Record Service (NCRS)

The National Care Record Service (NCRS) is the way in which health professionals now access the Summary Care Record (SCR). The SCR has not changed, it is just the way SCRs are accessed that has changed.
