Welcome to Lynwood Medical Centre

We provide comprehensive NHS primary care services to help you manage your health and well-being.

Our aim is to provide a high quality, caring and personal health care service to our whole patient population by:

  • Putting our patients at the centre of what we do
  • Having a highly qualified and trained multi-professional integrated primary Healthcare Team
  • Offering our services in a safe, supportive and suitably equipped environment, using technological advances in healthcare systems for our patient’s benefit.

Although there is no designated pram park it is requested that parents/ guardians leave prams/buggies in the porch/at the front of the building as access for prams/buggies is limited within the building.

There is no patient car parking on site however there is limited car parking available on the street and surrounding streets. Although there is no displayed disabled parking available, it is permitted to drop off in front of the surgery and allow disabled patients to alight vehicles at the front of the building.

If you’re a patient at our practice you can now use the new NHS App, a simple and secure way to access a range of NHS services on your smartphone or tablet.

Vaccination Campaigns & Coronavirus Update

Opt out of data sharing:

In order to register dissent for your children’s medical records to be used for purposes beyond their direct care:

1) Protect your GP data:  fill in and give this ‘Type 1’ form to your GP practice [PDF] – this form allows you to include details for your children and dependants as well. This is the most urgent step; the deadline to get your form to your GP practice is 23 June 2021according to NHS Digital.

2)  If you want to stop your non-GP data, such as hospital or clinic treatments, being used/sold for purposes other than your direct care (e.g. for “research and planning“) you must use this process:

    • If you have children under 13, you need to fill in this form [PDF] and e-mail or post it back to NHS Digital – this form works for both you and your children.
    • If you have an adult dependant for whom you have legal responsibility, you must use this form [PDF] and send it back to NHS Digital on their behalf
